New Member Waiver and Agreement
Acknowledgment and Initials

Payment Terms
- I understand that my membership will be billed monthly at {membership_fees}.
- I acknowledge a one-time sign-up fee of {signup_fee}, charged upon signing this agreement.
- My membership will commence on {start_date}.
Cancellation Policy
- To cancel my membership, I must provide a 14-day written notice prior to the next billing cycle.
Equipment Usage
- I agree to use the weight room only under coach supervision, in compliance with my membership terms.
Liability Waiver and Release
- I fully understand and acknowledge that physical exercise involves risks of serious injury, including permanent disability and death. I hereby agree to assume all risks associated with participation in activities at Orange County Regional Training Center.
- I release Orange County Regional Training Center, its employees, agents, and officers from all liability for any injuries, losses, or damages I might suffer in connection with my participation in any activities or use of equipment.
- This waiver and release extends to all claims of every kind or nature whatsoever, foreseen or unforeseen, known or unknown.
Refund Policy
- I understand that no refunds will be provided under any circumstances by Orange County Regional Training Center.
Automatic Renewal Clause
- My membership will automatically renew unless a 14-day notice is given prior to the renewal date.
Late Payment Terms
- Late payments will result in a $20 additional fee.
Membership Freeze Policy
- I may freeze my membership with proper notification or it will automatically renew, requiring a 14-day notice prior to the renewal date to avoid this.
Member Acknowledgment
By signing below, I affirm that I have thoroughly read, understood, and agree to be bound by all terms and conditions of this Agreement, including the liability waiver and release. I acknowledge that this agreement represents a release of liability and a waiver of my legal rights in connection with my participation in activities at Orange County Regional Training Center.
